The next chapter addresses DI and head injuries. Next, the management of neuroendocrine instability during maintenance of potential organ donors is described. Organ transplants have gradually increased worldwide. To have maintenance of appropriate potential organs, AVP is needed. Furthermore, nephrogenic DI-the potential therapeutic drugs and analysis of membrane protein stability is the topic of the next two chapters, followed by new insights into the diagnosis and management of pregnancy-related DI. The seventh chapter reports on the problems with differential diagnosis in a case of central DI in a female patient with bipolar disorder. The lithium treatment usually resulted in nephrogenic DI. Finally, over the last years, the development of MRI imaging on the pituitary gland with the stalk and hypothalamus has advanced. He has directed more than eighty undergraduate and graduate degree theses. He is a member of the scientific communities of beekeeping and entomology and has continued to organize more than twenty scientific congresses and seminars in his specialty. He is a reviewer for scientific journals and books. He has been president and director of different scientific societies and advisor to the Chilean Beekeeping Network, where he is also a consultant to the Latin American Beekeeping Federation for the congresses held in different countries. One way to achieve this objective is the use of prognostic and predictive factors; a prognostic factor provides prospective information on the evolution of the patient being able to guide therapeutic decisions, while a predictive factor gives us information on the probability of tumor response to a therapeutic agent.
\nThe characteristics that a prognostic factor must meet are: (a) simple prediction method, (b) wide availability, (c) sensitivity, and (d) reproducibility in any clinical situation. The purpose of these markers is to help define the individual prognosis of clinical groups, select patients who may need other treatments, and assign the most effective treatments to improve survival and quality of life.
\nAlthough currently the therapeutic decisions are still based on the classic clinical and pathological prognostic factors already known, such as age, functional status, sex, chest pain, weight loss, thrombocytosis, leukocytosis, anemia, and histological type [3, 10], biological and genetic factors may soon be excellent options as prognostic and predictive factors.
\nMultiple mesothelioma series have validated advanced TNM stage, age ≥ 50 years, male gender, poor performance status, weight loss, platelet counts ≥400,000, white blood cell counts ≥15.5, low hemoglobin level, low albumin levels, and high serum lactate dehydrogenase levels, among others, as poor predictive and prognosis factors [11, 12, 13, 14, 15, 16, 17, 18, 19, 20, 21]
TNM stage is one of the most studied prognosis factors describing a poor survival prognosis for those with advanced or metastatic stage, however, in the same stage of the disease, patients’ survival varies widely suggesting that TNM staging is not completely precise to predict a survival outcome [16]. Moreover, with the new changes applied since the release of the eighth edition of the TNM Classification for Lung and Pleural Tumors where all patients N0M0 malignant pleural mesothelioma as stage IA or IB, differing from the seventh edition classification, in which N0 also was listed within the classifications for stages II and III. These changes reclassified as stage I many patients who were formerly considered as stage II or III since some patients at stage IB experienced poorer prognosis than those at stage III [22, 23]. Identifying prognostic factors based on the new classification should help to identify the patients with a poor prognosis who may benefit from multimodality treatments. Additional to the TNM staging system, the true tumor volume was independently associated with overall survival and response to treatment; however, more studies need to be done to validate this variable [24, 25, 26, 27]
Previous studies have suggested that females with mesothelioma experience longer survival compared to males [6, 28, 29, 30, 31, 32, 33] with possible suggested explications like those they present at earlier stage [34], tumors with more favorable histology [30], different asbestos exposure responsible for a more indolent tumor biology [35], and a protective effect of circulating estrogen interacting with estrogen receptors present in their tumors, [32, 36, 37] however, only more indolent tumor biology associated to higher frequency of germline mutations in DNA repair genes [38, 39, 40, 41] and interaction of estrogens with estrogen receptor beta [36, 37, 42, 43]
Platelet count is a practical and easy blood test in clinical practice that has been studied for its role as a prognosis factor due to the interaction of platelets with tumor cells contributing to tumor progression, invasion, metastasis, and angiogenesis [44]. This interaction could be explained by five possible pathways: the first one refers to the release of growth factor by the platelets, including transforming growth factor β and fibroblast growth factor enhancing cancer cell proliferation [45]. Second, platelet membranes are rich in many adhesin molecules like selectins, integrins, immunoglobulin superfamily proteins, and leucine-rich glycoproteins stabilizing the cancer cell arrest in the vasculature, increasing potential of metastasis [46]. Third, platelets could mediate the invasive potential of cancer cells by the release of thromboxane A2, 12-hydroxyeicosatetranoic acid, and matrix metalloproteinases [47, 48, 49]. Fourth, platelets release a large number of pro-angiogenic mediators such as vascular endothelial growth factor and basic fibroblast growth factor influencing the tumor angiogenesis and consequently tumor growth [50, 51, 52]. Fifth, some studies have demonstrated that platelets facilitate the immune escape of cancer cells by surrounding tumor cells and protecting them from the cytotoxic effect of natural killer cells [53, 54]. Several studies concluded that thrombocytosis is correlated with worse overall survival in patients with mesothelioma, indicating that pretreatment could be an adequate and useful factor of prognosis [18]
Recently, many people have focused on the role of inflammation in cancer due to its contribution to tumor initiation and malignant progression. More specifically in mesothelioma, inflammation becomes relevant since most patients have a history of asbestos exposure, and this mineral can skewer cells and set off chemical reactions that lead to inflammation, DNA damage, and cell death [20]. Leukocyte blood count reflects a degree of the systemic inflammatory response in tumor patients, being a valuable and simple indicator [55]. Blood neutrophil-to-lymphocyte ratio is a systemic marker for inflammation closely related to the mortality rate and response to the treatment is useful as a predictive and prognostic factor, taking 3 as a dividing point [20, 56, 57, 58, 59, 60]. In the same way, serum c-protein can reflect an inflammatory environment; although its usefulness as a prognostic and predictive factor has been demonstrated in limited studies, more research is needed to validate its utility [61, 62, 63].
\nMalnutrition has been related to adverse outcomes in overall survival, quality of life, and increased mortality of malignant tumors [64, 65, 66]. Serum albumin level is a simple and objective indicator to evaluate malnutrition. Multiple studies have demonstrated hypoalbuminemia as an adverse independent prognostic factor for mesothelioma [19, 20, 67].
\nIt is well known that cancer cells tend to employ alternate metabolic pathways, generating adenosine triphosphate through anaerobic glycolysis regulated by lactate dehydrogenase [68, 69]. Several studies assessed the value of high pretreatment lactate dehydrogenase levels for the prediction of a worse survival outcome in mesothelioma [10, 61, 62, 70, 71, 72, 73, 74, 75]. The association between high lactate dehydrogenase levels and poor prognosis on malignancies has tried to be explained in multiple ways. The first theory implies that the production of lactate acid could be up-regulated by lactate dehydrogenase, generating an acidic environment activating metalloproteases, macrophage-mediated angiogenesis and protecting mitochondria from oxidative stress, which induces resistance to hypoxia-induced apoptosis of tumor cells [76, 77, 78, 79, 80]. The second theory explains a strong correlation between elevated lactate dehydrogenase levels and an up-regulation of the hypoxia-inducible factor pathway resulting in a host immunological function attenuation, and enhanced tumor angiogenesis, which has an adverse impact on prognosis in malignant tumors [81]. Despite the great evidence of the utility of lactate dehydrogenase as a convenient and cost-effective indicator for predicting overall survival outcome, cut-off values of lactate dehydrogenase reported on the literature are inconsistent, and it is important to standardize the cut-off value in future studies.
\nTogether with the TNM stage, the histological type is one of the strongest prognostic factors among patients with mesothelioma. However, with the support of immunohistochemistry markers, not only has diagnosis been improved, but also new markers have appeared for a more accurate prediction of response to treatment, overall survival, and developing better therapeutic approaches.
\nThe most significant prognostic factor until now remains histology with a better prognosis for epithelioid type than sarcomatoid or biphasic type mesothelioma [10, 12, 82, 83]. In addition to histologic subtyping (with solid growth pattern being associated with a poor outcome), nuclear atypia, mitotic count, and the presence of necrosis were found to be independent prognostic factors in epithelioid malignant pleural mesothelioma [84, 85, 86].
\nKi67 antigen is used for the assessment of growth fraction of cell populations, due to it being exclusively expressed in proliferating cells; cell cycle analysis showed that Ki67 is detectable in G1, G2, S, and mitosis phases but absent in quiescent cells [87, 88]. Despite most studies indicating that high expression of Ki67 leads to a poor prognosis, some malignancies showing high Ki67 levels actually show a better response to treatment, which could be explained by the fact that cells with high proliferation are susceptible to cytotoxic agents [89, 90, 91, 92, 93]. The detection of Ki67 is not a routine procedure for mesothelioma’s diagnosis and treatment; however, a group has suggested to consider it due to its utility as a possible prognostic marker in epithelioid mesothelioma with a better prognosis outcome in those with low expression levels [94, 95, 96, 97, 98]
Calretinin is a calcium-binding protein that has been established as a useful marker in distinguishing mesothelioma from adenocarcinomas with pleural metastases [99]; Additionally, interest in using higher calretinin scores as favorable prognostic factors has been growing, although further investigation is needed [100, 101, 102, 103, 104]
As mentioned above in the section of clinical factors, estrogen receptor beta expressed on mesothelial tumor cells has become a promising prognostic factor and a possible future therapeutic target [36, 37, 42, 43]
Several biomarkers are selectively elevated in patients with mesothelioma. However, further study and validation are required before they are recommended as routine predictive or prognosis factors and they should be adjunct to a radiological assessment. With considerable variation in response to treatment, the emergence of promising biomarkers that could select responders from non-responders at baseline or during treatment would guide to a better therapeutic approach, prevent patients from getting ineffective treatments, and improve cost-effectiveness.
\nThe most researched biomarker until now is the mesothelin; soluble mesothelin is a circulating form of a membrane-bound glycoprotein highly expressed by mesothelial cells in mesothelioma (predominantly epithelioid type) and other malignancies [105]. Despite the controversial evidence reported in the literature [106, 107, 108, 109, 110, 111, 112, 113, 114], a meta-analysis conducted by Tian et al. [115] concluded that a high soluble mesothelin level may lead to a poor prognosis for patients with mesothelioma, it being appropriate to consider mesothelin level as an independent prognostic marker.
\nHuman fibulin-3 is a secreted glycoprotein that plays an essential role in the regulation of cell proliferation and migration [116, 117]. Recent findings have documented altered levels on patients with mesothelioma, highlighting them as a novel biomarker for this malignancy; however, as most studies have been done with limited sample size [114, 118, 119, 120], and the results may not completely mirror the actual value of fibulin-3 for prognosis, further studies are needed for a more comprehensive prognostic role of human fibulin-3 in mesothelioma.
\nOsteopontin is a glycoprotein that mediates cell-matrix interactions with adverse outcomes for mesothelioma [98, 121, 122]; however, its utility is limited because of the significant variability in the cut-offs used between studies. In order to be validated in the future, a consensus approach is required for sampling and analysis [122]
CA 125 is a transmembrane glycoprotein that can be detected in the fallopian tube, endometrium, endocervix, and mesothelial surface of the peritoneum, pleura, and pericardium [98]. Some cases with non-gynecological cancer showed positive immunohistochemical staining for CA125 in tumor tissue and elevated CA 125 levels in serum [123, 124, 125]. The baseline levels of serum CA125 accompanied by the stage of the disease could be used as independent prognostic factors for patients with mesothelioma; the change in serum CA125 levels can predict overall survival and response to systemic treatments [126, 127, 128]
The best-known clinical prognostic scoring systems for mesothelioma until now derive from the Cancer and Leukemia Group B (CALGB) and the European Organization for Research and Treatment of Cancer (EORTC), both scores have been widely used to better select patients who have a favorable prognosis and could tolerate and potentially benefit from a more aggressive combined modality treatment [3, 10]
The CALGB index was validated by examining the survival of a wide cohort dividing patients into six patient subgroups with different survival rates. The CALGB study considered extent pleural disease, lactate dehydrogenase >500 UI/L, poor performance status, platelets >400,000, non-epithelial histology, and >75 years as negative prognostic factors for survival. The most favorable characteristics were a performance status of 0, age < 49, and hemoglobin ≥14.6/μl [10]
The EORTC score has been validated in 523 patients included in 10 mesothelioma trials with the analysis suggesting that performance status >0, stage IV disease, and biphasic or sarcomatous histologies are associated with a worse outcome [129]. Additional reports confirmed that male sex, older age, and abnormal hematological values also give a poor prognosis [13, 130].
\nDespite both studies identifying performance status and histology as two main prognostic factors, these analyses included patients with heterogeneous tumor stages at diagnosis, the majority of whom underwent major surgery and whose treatment predated the use of pemetrexed as first-line treatment. Since the positioning of pemetrexed as a first-line treatment, no validated prognostic score has appeared, resulting in the need to generate new studies with the aforementioned scores [131].
\nAlthough there are multiple prognostic and predictive factors that are currently validated, many others have generated great interest for their potential as a therapeutic target in the future.
\nThere is an increasing interest in the use of semi-quantitative 18F-FDG PET/CT parameters, like metabolic tumor volume and total lesion glycolysis to measure the metabolic activity in the entire tumor volume with great potential to predict response to treatment [119, 132, 133, 134, 135, 136, 137, 138, 139, 140, 141, 142, 143, 144]; however further investigation is needed in mesothelioma patients.
\nDespite the wide utility of the tissue biopsy, the invasive nature limits their application, especially when repeated biopsies are needed. Given the aforementioned, liquid biopsy has gained interest from oncologists and basic researchers [145]. Although liquid biopsy is still far from replacing tissue biopsy for mesothelioma, plasma and serum samples represent minimally invasive, low-risk, and easily obtained biological fluids that many studies have indicated as potentially interesting prognosis biomarkers as mentioned in the section “Biological factors” [146].
\nNowadays, immunotherapy is gaining great relevance in cancer therapeutics. Soon, oncologists will routinely ask for programmed death-ligand 1 (PD-L1) status that has been correlated with better treatment response to anti-PD-L1 antibodies and overall survival outcomes [147, 148, 149, 150, 151]. However, different PD-L1 antibodies coupled with specific staining platforms and scoring criteria may be necessary since finding a suitable cut-off point remains a current challenge [151, 152].
\nA wide number of molecular prognostic markers for mesothelioma have been investigated. The number of tumor-infiltrating myeloid cells, c-MET expression, thymidylate synthase expression, among others, represent promising biomarkers associated with strong prognostic significance. c-MET is a tyrosine kinase receptor, its overexpression was associated with longer overall survival in patients with mesothelioma [98, 153]. Thymidylate synthase expression may predict pemetrexed efficacy, a certain correlation has also been found with overall survival and progression-free survival [154]
Dysregulated genes play a critical role in the development and progression of mesothelioma, making them future diagnosis and prognosis biomarkers [155]. Recently, Zhou et al. obtained an RNA-Seq count quantified by RSEM for RNA expression profiles of a large cohort of patients with mesothelioma according to The Cancer Genome Atlas guidelines. After a time-dependent receiver operated a characteristic curve to evaluate the prognostic performance of survival prediction, three genes (LSM6, GZMB, and HJURP) were found with a strong statistically significant prognostic association; this prognostic signature could be a clinically useful tool that in the future could be incorporated into a clinical sequencing program to individualize therapy [156]
Despite the wide variety of predictive and prognostic factors that exist, just a few are replicable worldwide. Furthermore, only pathological type and performance status are the grade-A recommendations of prognostic factors in pretreatment assessment, as well as the nodal stage, residual disease, and histology during treatment [16].
\nAlthough there is currently no validated prognostic approach, according to individual evidence, availability, and cost-benefit, it is recommended to pay special attention to the TNM classification, histological type, and serum CA125 in the decision for multimodal therapy. Despite the practicality of the prognostic scoring systems, further investigations are needed to validate the known scores or generated new ones that fit the new existing therapeutic modalities for mesothelioma.
\nIn the near future, many other prognostic and predictive factors may be introduced in clinical practice making a selection of mesothelioma subgroups to improve the benefit achievable by currently available treatment strategies, and relentless efforts will have to be focused on designing innovative compounds selectively targeting the existing (or additional) markers to improve the grim prognosis of the disease.

\nAcute kidney injury (AKI) is a frequent complication in several clinical settings, including large surgeries [1], emergency departments [2, 3], and intensive care units (ICUs) [4]. The incidence of AKI has been increasing over the years, with about 2 million people being affected in 2010 [5], despite the efforts of researchers and organizations [6, 7, 8]. AKI is commonly followed by worse outcomes: prolonged length of ICU and hospital stay, need for dialysis, decreases in the glomerular filtration rate (GRF), development of chronic kidney disease (CKD), and increases in mortality [9, 10, 11].
In recent decades, therapeutic interventions aimed at reversing kidney dysfunction have had disappointing results in multiple settings; thus, the research focus has shifted from treatment to prevention and early detection by focusing on two main issues: diagnostic criteria and early diagnosis. In 2004, the Acute Dialysis Quality Initiative (ADQI) sought a more uniform definition of AKI, and the most recent consensus definition was published in 2012 by Kidney Disease: Improving Global Outcomes (KDIGO) [8]. The diagnosis of AKI is based on changes in serum creatinine (Scr) and urine output (UO), but neither marker is kidney specific. Efforts have been made to identify novel biomarkers that have high sensitivity and specificity.
The standardization of the diagnosis of AKI allowed us to compare the diagnoses made in different settings. However, the issue of early diagnosis is still a challenge. First, there are limitations regarding the use of Scr and UO. Second, determining the need for renal replacement therapy is difficult due to a lack of information about whether the AKI is transient or persistent. Last, the research advances identifying early biomarkers have thus far been inaccessible in clinical practice [12].
Neutrophil gelatinase-associated lipocalin (NGAL) [13] has been far and away the most promising biomarker to help fill this gap, and its diagnostic capabilities as a biomarker have been confirmed in a large number of clinical trials. This chapter aims to present information on the role of NGAL in the renal injury process, its expression in the kidney, confounding factors, the type of assay used, whether plasma or urine NGAL has better accuracy, the cutoff values in normal individuals, the accuracy of NGAL for diagnosing AKI, and the evaluation of other outcomes.
In 1993, Kjeldsen et al. [13] isolated lipocalin as a protease-resistant polypeptide covalently bound to neutrophil gelatinase, named neutrophil gelatinase-associated lipocalin (NGAL), also known as siderocalin, lipocalin 2 or oncogene 24p [14].
NGAL is a 25 kilodalton (kDa) protein covalently bound to gelatinase in neutrophil-specific granules. NGAL is expressed at very low levels in various human tissues, including the kidneys, uterus, prostate, salivary gland, trachea, lungs, stomach, and adult and fetal colon [15, 16]. The anti-inflammatory function of NGAL is demonstrated by increased NGAL expression in proliferative epithelia, inflammatory areas, and intestinal malignancies [17].
In normal kidneys, the expression of NGAL is mainly released by the thick ascending limb and the intercalated cells of the thick collecting duct. Some NGAL expression is also present in the proximal tubular epithelium, once NGAL is filtered by the glomerulus and reabsorbed by the proximal tubule in a megalin-dependent manner [17, 18]. The physiological function of NGAL in the kidneys is unknown; however, the role of NGAL in renal morphogenesis is under consideration [19]. NGAL also has a predominant role in the regulation of cell proliferation, repair processes, and tubular reepithelization. NGAL expression corresponds to an additional iron transport pathway, which increases the transcription of hemeoxygenase, an enzyme with proliferative and antiapoptotic effects that protects and preserves proximal tubular cells [20, 21].
Several biological functions for NGAL have been suggested; in the kidney, NGAL release is associated with ischemic or nephrotoxic insults. Additionally, a decrease in tubular reabsorption after AKI may lead to a further increase in urinary NGAL concentration, resulting acquiring a status of the “troponin” of the kidneys [22, 23, 24].
The conditions that can interfere with the performance, sensitivity, and specificity of NGAL, already identified as a biomarker, are sepsis, chronic obstructive pulmonary disease, and cardiac dysfunction, and the presence of these conditions may act as confounding factors for NGAL measurements. The predictive performance of NGAL seems to also be influenced by age (higher predictive value in children than in older patients), sex (higher predictive value in female patients than in male patients), urinary tract infection, and impaired renal function (higher predictive value in patients with chronic kidney disease) [25, 26, 27].
Sepsis will be more thoroughly addressed in the next chapter.
The commercialization of NGAL as the gold standard for the diagnosis of AKI is somewhat controversial [28]. There are many types of NGAL assays available on the market that use different nonautomated ELISA platforms, which makes it difficult for comparisons to be made among studies from around the world.
The first kit for the quantitative and automated determination of NGAL by the ELISA method was developed by Abbott Laboratories (Abbott Park, IL, USA) for the urinary evaluation of NGAL, with a cutoff value of 141 μg/L (95% CI 125–158 μg/L). An EDTA plasma blood test was created by the Triage Meter platform (Biosite-Inverness Medical, Waltham, USA) with a cutoff value of 163 μ/L (CI 109–221 μ/L) [29].
Bioporto (Bioporto Diagnostics A/S, Gentfte, Denmark) developed a new particle-enhanced turbidimetric immunoassay (PETIA) that has the advantages of flexibility (adaptation for clinical use in different analyzers), automation (closer to clinical practice), and applicability in different biological matrices (urine and plasma) [30].
The chemiluminescence test is an alternative to assess NGAL, and it is commonly used to analyze studies with small animals since it is possible to do the analysis with few substrates [31].
Because there are three known molecular forms of NGAL, the assay of interest should differentiate the 25 kDa NGAL monomer produced by the monocyte tubular epithelial cells from other forms of NGAL: 45 kDa NGAL, from the homodimer predominantly secreted by neutrophils, and the 145 kDa NGAL/matrix metalloproteinase-9 (MMP9) covalently complexed heterodimer [32, 33].
According to Mårtensson et al. [34] and Cai et al. [33], the combination of two ELISAs, may improve the diagnostic accuracy of NGAL, one to determine the monomeric form and the other to determine the homodimeric form.
The confounding factor of sepsis is described herein and is dependent on the assay method, as well as whether the chosen kit is less sensitive to 25 kDa NGAL expressed by tubular epithelial cells. The test can also measure the 45 kDa homodimer predominantly secreted by neutrophils, which are common in sepsis, and the increase in neutrophils increases homodimeric NGAL expression and results in false positives.
The consensus is clear that NGAL measured in urine has better performance [35], because the release and increase in NGAL will occur first in urine. However, the collection of urine depends on the urine output, which is sometimes not available. Some benefits of plasma NGAL are that it is available at any time and is more accurate in anuric or oliguric patients.
The issue about the normalization of urinary NGAL by correction for the urinary creatinine level is debatable, but it has been used to correct for urine output in cases of oliguria or pollakiuria, avoiding inaccurate concentration or dilution measurements of the biomarkers. The fact is that the creatinine release time is different from the biomarker release time, and the normalized level will be affected by this difference and will not represent a real physiological value [36].
The determination of normal NGAL levels in healthy adults has been inadequately described in the literature. However, some studies have been performed, such as that of Cullen et al. [26], which analyzed urine by the Abbot-Architec assay in 174 healthy people (100 men and 74 women aged between 19 and 88 years). The value of the immunoassay result was normalized by urinary creatinine, and the cut-off value was 107 μ/mmol (13 μ/mmol). There was a higher concentration of creatinine-normalized urinary NGAL in women, in the elderly and in patients with leukocyturia.
The study reported by Stejkal et al. [37] analyzed BioVendor’s NGAL assay using the serum of 136 healthy, nonobese individuals (53 men and 83 women). The authors reported median NGAL values (78.8 μg/L for men and 80 μg/L for women). Pernnemans et al. [27] analyzed the NGAL ELISA (RD System Europe, Abingdon, UK) and other urinary biomarkers in 338 healthy individuals (199 women and 139 men, aged 0 to 95 years). They reported that the NGAL reference range of the 21–95 years age group was 73.88–211.16 μg/L in women and 149.26–182.58 μg/L in men, and there was higher expression in elderly individuals.
An NGAL PETIA (Bioporto Diagnostics A/S, Gentfte, Denmark) [36] was evaluated for 200 healthy nonobese individuals (137 men and 63 women, with a mean age of 39 years (SD 11.2)). They proposed reference plasma concentrations from 38.7–157.6 ng/ml for women and 24.4–142.5 ng/ml for men and proposed reference urine concentrations of <9–54.5 ng/ml for both sexes. The authors reported that the mean values in men were higher than in women, 78.9 ng/ml vs. 73.8 ng/ml, respectively; there was a significant difference in NGAL in relation to age.
In addition to the variability of the chosen immunoassay, the unit of measurement in the interpretation of the NGAL studies should be considered—the most commonly found are ng/ml, μg/L, ng/dl, mg/ml and μg/mmol.
NGAL is the most widely investigated AKI biomarker. Its performance for predicting AKI has been evaluated in various settings, such as in pediatric and adult cardiac surgery patients, in critically ill patients, and in patients in the emergency room, as well in kidney transplant and other settings [38, 39].
Numerous studies have demonstrated the ability of NGAL to diagnose AKI. For example, the study reported by Constantin et al. [40], that evaluated the plasma NGAL of 88 patients at ICU admission, found a sensitivity of 82%, specificity of 97% and AUC of 0.92 to cut-off value of 155 mmol/L to predictor of AKI.
A multicenter study, reported by Di Somma et al. [41], with 665 patients admitted to the emergency department, assessed plasma NGAL in several points after admission. Serial evaluation of NGAL at times zero and six hours provided a high negative predictive value (NPV) (98%) to rule out the diagnosis of AKI within six hours of the arrival of patients to the emergency department. The NGAL value at admission could demonstrated a strong predictive value for in-hospital mortality of the patient, with a cut-off value of 400 ng/ml.
In the meta-analysis by Haase et al. [42]—with 19 studies, totaling 2538 patients, of whom 487 (19.2%) developed AKI—NGAL was demonstrated to have diagnostic and prognostic value for AKI, with an OR of 18.6 (95% CI 9–38.1) and AUC of 0.81 (95% CI 0.73–0.89). The cut-off value ranged from 100 to 270 ng/ml, but a value of 150 ng/dl was suggested for the diagnosis of AKI.
In another recent meta-analysis by Zhou et al. [39]—with 24 studies, a total of 4066 patients from 9 countries, including studies with serum and urinary NGAL—the sensitivity for the diagnosis of AKI was 0.68 (95% CI, 65–0.70), and the specificity was 0.79 (95% CI 0.77–0.80).
In the study by Singer et al. [38], urinary NGAL was useful for classifying and stratifying patients with established AKI: the level of NGAL>104 μg/L indicated intrinsic AKI (odds ratio of 5.97), while the level of NGAL <47 μg/L indicated unlikely intrinsic AKI (odds ratio of 0.2). In the logistic regression analysis, NGAL was able to predict the worsening of the RIFLE class, the need for RRT and in-hospital mortality. The performance of NGAL to evaluate other outcomes will be discussed in the next chapter.
The study by Bennett et al. [43] clearly indicated that urinary NGAL is a powerful early biomarker of AKI after cardiopulmonary bypass that preceded the increase in serum creatinine by 2–3 days. Studies have shown that elevation of NGAL is detectable after 3 hours and peaks approximately 6–12 hours after injury. The elevation can persist up to 5 days according to the severity of injury [44, 45, 46]. In addition to Benett’s study, other studies in general have failed to reach conclusions about the early timing diagnosis of NGAL, which is the main finding required to reach a therapeutic window and better evaluate future medication targets in AKI.
Several studies have assessed the diagnostic value of NGAL to predict AKI, but only a few have analyzed the early diagnosis in hours/days and compared it with serum creatinine, as seen in the last chapter. Still fewer studies have evaluated the predictive performance of NGAL in other outcomes, such as the need for RRT, recovery of renal function, progression to end stage renal disease (ESRD) and mortality, which will be discussed in this chapter.
In the meta-analysis by Hall et al. [47], for 91 kidney transplant patients, the incidence of need for RRT was 4.3%, and NGAL, in this scenario, had an OR of 12.9 and AUC of 0.78. In the same study, NGAL and urinary IL18 were predictors of the need for RRT up to 1 week after transplantation. NGAL presented a good AUC of 0.81 (95% CI 0.70–0.92) 6 hours after transplantation and was also a predictor of graft recovery for up to 3 months.
In the study conducted by Constantin et al. [40], the cutoff value of NGAL to assess the need for RRT was 330 mmol/L. The value of urinary NGAL (Architect, Abbot Park, IL) was correlated with the need for dialysis (r: 0.48 P: 0.01), presenting an AUC of 0.86 2 hours after cardiopulmonary bypass in children [43].
A recent meta-analysis by Klein et al. analyzed 12 studies to predict the need for RRT and found an AUC of 0.70 (95% CI 0.63–0.80) for NGAL [48].
Bhavsar et al. [49] concluded that higher levels of NGAL (measured by the Luminex assay) were associated with stage 3 CKD incidence. Some researchers have discussed whether the association of NGAL level is not exclusively related to the increase in neutrophils already described by Tian et al. [50] and maintain that further studies would be needed to elucidate this issue.
In the meta-analysis by Haase et al. [42], the incidence of mortality was 5.4%, and NGAL, in this scenario, showed an OR of 8.8 and AUC of 0.70.
In the Ariza study [51], PNGAL and UNGAL were demonstrated to be strong predictors of prognosis, and UNGAL was significantly predictive of the MELD score using the 28-day mortality score AUC of 0.88 (0.83–0.92).
In the study by Bennett et al. [43], the value of urinary NGAL (Architect, Abbot Park, IL) was also correlated with mortality (r: 0.53 p 0.01), with an AUC of 0.91, 2 hours after cardiopulmonary bypass in children.
The study by Dent et al. [52], using PNGAL (Biosite Inc., San Diego, USA) in 120 children undergoing cardiopulmonary bypass (CBP) and a cut-off value of 150 ng/ml and AKI prediction, found an AUC of 0.96 2 hours after CBP. PNGAL was also strongly correlated with the duration of AKI (r = 0.57, p < 0.001) and hospital stay time (r = 0.44, p < 0.001), and PNGAL at 12 hours was correlated with mortality (r = 0.48, p: 0.004).
In the study by Daniels et al. [53], PNGAL (Alere Inc., Waltham, USA) was measured in 1393 adult patients with cardiovascular disease (CVD) who were followed for 11 years. Of these, 436 did not survive, and 169 died from CVD. PNGAL was a predictor of CVD mortality, with a risk ratio of 1.33% and a risk ratio of 1.19% for all causes of mortality.
This brief review, based on accumulated evidence, discussed the role and value of NGAL in the diagnosis and prognosis of AKI. Studies’ findings suggest that induction of NGAL plays an important role in kidney function preservation, reducing apoptosis, and enhancing proliferative responses. In kidney injury, rapid and massive upregulated synthesis of NGAL occurs in the distal tubule, which quickly increases the concentration of NGAL in urine [54]. In addition, other important considerations have been provided as “key point” to help researchers move the NGAL analysis to clinical practice as soon possible.

The sample includes 188 participants over 60 years of age, divided in 2 groups: healthy subjects (MMSE ≥28), without cognitive complaints, and individuals with CIND (MMSE between 24 and 27 and subjective cognitive complains). Drawing of cube and drawing of house, Benton Visual Retention Test (BVRT), and Block design are used to test the hypothesis that short visuoconstructive and visuospatial tests can distinguish normal from pathological cognitive aging in its very early stages. Results proved the discriminative sensitivity of BVRT general assessment criteria and of omissions and distortions in CIND. The diagnostic sensitivity of a modification of Moore and Wike [1984] scoring system for house and cube drawing tasks was confirmed as well. Drawing of cube and house could be used for quick screening of CIND in subjects over 60. Principal component analysis with oblimin rotation was performed to explore the different dimensions in the visuospatial and visuoconstructive abilities in old age. The aim of the programme was to collect knowledge about frail elderly, to assess their needs and to provide person-centred and integrated care better suited to their needs. The budget of EUR 88 million was provided by the Dutch Ministry of Health, Welfare and Sports. Putting the needs of elderly people at the heart of the programme and ensuring their active participation were key to the programme’s success. The programme outcomes included the establishment of eight geriatric networks around the medical universities with 650 organisations and the completion of 218 projects. These projects, involving 43,000 elderly people and 8500 central caregivers, resulted in the completion of 45 PhD theses and the publication of more than 400 articles and the development of 300 practice toolkits, one database and a website, www.beteroud.nl. The Dutch National Care for the Elderly Programme has since developed into a movement and continues under the consortium Ageing Better, made up of eight organisations. This research aims to take a step forward in the study of narrative processing, by evaluating TS in a sample that helps to detect this deficit when it is neurogenic and recently manifested. The relationship between TS, secondary to focal brain injury, and neuropsychological and neuroanatomical variables was explored. A comprehensive neuropsychological battery was administered to 175 volunteers: 95 alert inpatients, without aphasia, without psychiatric history and without TS history, and 80 healthy participants, without TS. Results: TS (prevalence 16%) was independent of type or site of injury. An adverse effect of TS on global neuropsychological performance was observed. This effect was significantly related to attentional errors along with prolonged processing times but not to correct responses. Reliability and validity indices for the present TS screening scale were provided. Conclusion: Present results support the hypothesis that this neurogenic inability to spontaneously find, organize and communicate verbal information, beyond single words, depends on extended brain networks involving processes such as sustained attention, complex-syntax comprehension, the (implicit) interpretation and spontaneous recall of a narrative, and emotional and behavioral alterations. They become easily objects for lust, excluded from the beauty of graceful existence and noble courage. It is argued that the naked human body is not designed for pornographic exploitation and promiscuous sensuality but for compassionate intimacy and nurturing care in order to instil a humane dimension in human and sexual encounters. Previous studies have shown that hnRNPA1 had reduced protein levels and increased cytoplasmic accumulation in senescent human diploid fibroblasts. The consequence of reduced protein expression and altered cellular localization may account for the alterations in gene expression observed during senescence. There is limited information for gene targets of hnRNP A1 as well as its in vivo function. In these studies, we performed RNA co-immunoprecipitation experiments using hnRNP A1 as the target protein to identify potential mRNA species in ribonucleoprotein (RNP) complexes. Using this approach, we identified the human double minute 2 (HDM2) mRNA as a binding target for hnRNP A1 in young and senescent human diploid fibroblasts cells. It was also observed that alterations of hnRNP A1 expression modulate HDM2 mRNA levels in young IMR-90 cells. We also demonstrated that the levels of HDM2 mRNA increased with the downregulation of hnRNP A1 and decrease with the overexpression of hnRNP A1. Although we did not observe a significant decrease in HDM2 protein level, a concomitant increase in p53 protein level was detected with the overexpression of hnRNP A1. Our studies also show that hnRNP A1 directly interacts with HDM2 mRNA at a region corresponding to its 3′ UTR (untranslated region of a gene). The epithelial, dermal, and subcutaneous compartments are heterogeneous mixtures of cell types, yet they all display age-related skin dysfunction through the accumulation of an altered phenotypic cellular state called senescence. Cellular senescence is triggered by complex and dynamic genetic and epigenetic processes. A senescence steady state is achieved in different cell types under various and overlapping conditions of chronological age, toxic injury, oxidative stress, replicative exhaustion, DNA damage, metabolic dysfunction, and chromosomal structural changes. These inputs lead to outputs of cell-cycle withdrawal and the appearance of a senescence-associated secretory phenotype, both of which accumulate as tissue pathology observed clinically in aged skin. This review details the influence of genetic and epigenetic factors that converge on normal cutaneous cellular processes to create the senescent state, thereby dictating the response of the skin to the forces of both intrinsic and extrinsic aging. \r\n\tThe environment is subject to severe anthropic effects. Among them are those associated with pollution, resource extraction and overexploitation, loss of biodiversity, soil degradation, disorderly land occupation and planning, and many others. These anthropic effects could potentially be caused by any inadequate management of the environment. However, ecosystems have a resilience that makes them react to disturbances which mitigate the negative effects. It is critical to understand how ecosystems, natural and anthropized, including urban environments, respond to actions that have a negative influence and how they are managed. It is also important to establish when the limits marked by the resilience and the breaking point are achieved and when no return is possible. The main focus for the chapters is to cover the subjects such as understanding how the environment resilience works, the mechanisms involved, and how to manage them in order to improve our interactions with the environment and promote the use of adequate management practices such as those outlined in the United Nations’ Sustainable Development Goals.
