The DOEs with small feature sizes can be fabricated by the direct-writing electron-beam lithography.(Kley, 1997) To predict the electron dosage to obtain a desired resist profile, the effects of electron scattering in a resist layer and resist characteristics of development process must be considered.(Neureuther et al., 1979) Determining the optimum electron dose is called the proximity correction.
\n\t\t\tSome methods of the proximity correction were proposed,(Maker & Muller, 1992; Daschner et al., 1995; Nikolajeff et al., 1995) which are classified according to estimation methods of resist profile. One popular method is based on a point-spread function of the electron scatter and a resist contrast curve.(Daschner et al., 1995) The point-spread function is the density distribution of absorbed energy for a focused electron-beam. The function can be assumed to be the sum of two Gaussian distributions due to forward scattering and back scattering. The absorbed energy density is expressed by a convolution of the electron dose distribution and the point-spread function. The resist profile after developing is estimated from the absorbed energy density and a resist contrast curve. The resist contrast curve shows the remaining resist thickness as a function of the absorbed energy density. The point-spread function and the resist contrast curve are determined experimentally. This estimation method is widely used because of the simple process of calculation. However the contrast curve method is not valid for short-period gratings with deep grooves, because the absorbed energy density is not uniform vertically.(Okano et al., 2000) We had reported a precise proximity correction method based on the electron-beam lithography simulator,(Hirai et al., 2000) which consists of an electron scatter simulation by the Monte Carlo method(Kyser & Murata, 1974) and a resist development simulation with the cell removal model.(Dill et al., 1975) This correction method is suitable for short-period gratings and chirped period diffraction gratings such as a diffractive lens.(Okano et al., 2003)
\n\t\t\tDiffraction efficiencies of the periodic diffraction gratings can be calculated by numerical analysis such as the rigorous coupled wave analysis (RCWA) method,(Moharam & Gaylord, 1981) the differential method,(Vincent, 1980) and the finite difference time domain (FDTD) method.(Yee, 1966) The diffraction efficiency is defined as a ratio of an incident and the diffracted beam powers. It is known that when the grating period is several times as long as the light wavelength (<5λ), the saw teeth profile is not the solution to obtain the highest efficiency of the first order diffracted light.(Shiono et al., 2002) There are some reports to calculate the grating profile to obtain the highest diffraction efficiency using the rigorous grating analysis.(Noponen et al., 1992; Sheng et al., 1997; Kallioniemi et al., 2000; Testorf & Fiddy, 2001) The diffraction efficiencies of the optimized gratings were taken very high values. Noponen et al. found the grating profile with the efficiency of 85.7% in case the diffraction grating has a period of 2.5λ.(Noponen et al., 1992) If the grating profile is the simple saw teeth, the efficiency will be ~45%. However, such the short period gratings are not easy to be fabricated with the direct-writing electron-beam lithography, because the optimized grating profiles have sharp peaks and narrow valleys. The proximity effect of electron scattering restricts the grating profiles. A small difference of the grating profile from the designed one causes a decrease of diffraction efficiency.

In this chapter, we demonstrate a unified design method optimizes the electron dose and grating profiles simultaneously to obtain the desired diffraction efficiency under the restriction of the proximity effect.(Okano et al., 2004; 2007) The optimization is made with the rigorous electromagnetic wave analysis and the resist development simulator. We design two cases of short periodic diffraction grating and the diffractive lens using the unified design method.

Several parameters of the electron-beam resist are required for the lithography simulation. The resist parameters must be determined from a primary experiment.(Okano et al., 2003) We used the resist parameters of OEBR-1000 (Tokyo Ohka Kogyo) for the simulation. An electron-beam resist of 1.7 μm thickness is coated on a 1.0-mm-thick silica glass plate. The acceleration voltage of the electron beam is 50 kV, and the development time is 120 s. We designed a short period diffraction grating of the electron-beam resist. The grating period is 1.4 μm. The refractive indices of the resist and substrate were 1.49 and 1.46, respectively. The light wave of 633 nm wavelength was normally incident onto the grating, and the light wave is polarized parallel to grating grooves. The resist profile was optimized to obtain the maximum diffraction efficiency for the +1st order diffraction wave. For such a short grating period, the saw-teeth blazed profile does not yield the highest diffraction efficiency.
\n\t\t\t\t\n\t\t\t\t\tFigure 2 (a) shows the grating profile designed with the unified method. The calculated diffraction efficiency is 67%. It took about 3 hours to optimize the dose profile by a computer with Compaq Alpha21264 (667 MHz) CPU. The number of iteration was 1000.
\n\t\t\t\tWe have also designed the grating profile with other designed methods. Figure 2 (b) shows the grating profile optimized only with the RCWA, i.e. the optical optimization process. And the calculated diffraction efficiency is 82%. This diffraction efficiency is higher than that of the suggested optimizing method. However, the grating profile is not easy to be fabricated accurately because of sharp peaks and narrow valleys. Figure 2 (c) is the grating profile designed with the series process, that is, the profile has been optimized under the restriction of the proximity effect as the object profile is Fig. 2 (b). The diffraction efficiency is reduced to 48%, which is about 30% lower than that of Fig. 2 (b). A conventional saw-tooth blazed grating shown as Fig. 2 (d) has the diffraction efficiency of 42%. It is obvious that the grating profile designed with the optimal design method (Fig. 2 (a)) is easy to be fabricated compared with that of Fig. 2 (b). And the diffraction efficiency is superior to those of the saw-tooth blazed grating and the series method.

\n\t\t\t\t\tFigure 3 shows the grating profiles designed with the unified method for different grating periods, (a) 1.0 μm, (b) 1.4 μm, (c) 2.6 μm and (d) 6.5 μm, respectively. For the long period grating shown as Fig. 3 (d), the profile becomes the saw-tooth-like blazed profile. On the other hand, for short period gratings, although the optimized grating profile has the round valleys, the diffraction efficiencies are higher than those of the saw-tooth blazed gratings.

The diffraction gratings designed in the previous descriptons have been fabricated with the direct-writing electron-beam lithography. The grating periods were 1.0 μm and 1.4 μm, that is, the grating profiles were Fig. 3 (a) and (b), respectively. We used the electron-beam writer JEOL JBX-5000SI with the acceleration voltage of 50 kV and the beam diameter of 20 nm. The resist thickness was 1.7 μm and the development time was 120 s. The silica glass substrate was 1.0 mm in thickness.
\n\t\t\t\t\n\t\t\t\t\tFigure 4 (a) and (d) show the electron-dose profiles calculated with the unified method for the grating periods of 1.0 μm and 1.4 μm, respectively. Though the difference of periods is 0.4 μm, the estimated dose profiles are so different from each other. In the actual fabrication, the electron dosages were according to the calculated dose profiles. The scanning electron microscope (SEM) pictures of the fabricated resist gratings are shown in Fig. 4 (c) and (f). Their surface profiles measured with the atomic force microscopy (AFM) are shown in Fig. 4 (b) and (e), respectively. The grating profiles calculated with the unified method are shown in the figures, too. The grating heights of the AFM profiles are agreed well with those of the calculated profiles. The maximum differences of the profiles are ~50 nm. The measured diffraction efficiency was 70% for the 1.0 μm-period grating, and 58% for the 1.4 μm-period grating. These efficiencies are lower by ~10% than the calculated efficiency.
\n\t\t\t\tThe decrease in diffraction efficiencies is caused by small difference of the grating profiles. In a case of the 1.4 μm-period grating, the diffraction efficiency calculated from the AFM profiles is lower by ~4% than the designed efficiency. The other reason is the stitching error of subfields in the electron-beam writer. The stitching error decreases by a few percents of the diffraction efficiency. To fabricate the grating profile accurately, the resist parameters must be determined more precisely. And the development process also has to be controlled carefully. The stitching error will be reduced by tuning the electron-beam writer.

In this section, we designed the diffractive cylindrical lens of an electron-beam resist using the unified method. The diffraction pattern on the focal plane is calculated from the resist profile by the FDTD method. Figure 5 shows the calculation model of the diffractive lens. (

The grid size of FDTD calculation is 50 nm square. The calculation area consists of 1199×109 cells and is surrounded by perfectly matched layers. The electron dose is modulated by 9-58 levels, which depends on the grating period. The calculation time for one process from giving the dose profile to evaluating the diffraction efficiency is 11 min. After 120 iterations, the diffraction efficiency practically converges. The total calculation time for the optimization is about 22 h using a computer with an AMD Athlon 1.2 GHz CPU.
\n\t\t\t\t\n\t\t\t\t\tFigure 6 shows the results of the optimal design. The dose and calculated resist profiles are shown in (a) and (b), respectively. The dose profile is complex and the calculated profile is very different from the conventional wrapped parabolic surface profile. Figure 6 (c) shows the calculated intensity distribution on the focal plane. The focus efficiency is 49%. Figure 7 shows the conventional wrapped parabolic surface grating and its intensity distribution. Its focus efficiency is 37%. The efficiency of the optimized diffractive lens is significantly higher than that of the conventional diffractive lens. The focus efficiency of the optical optimization without the lithography simulation is 53%.

We suggested the unified design method of grating profiles with the electromagnetic grating analysis and the electron-beam lithography simulator. The unified method gives us the optimum grating profile and the electron dose profile under the restriction of the proximity effect of the electron scattering. This method is useful especially for short-period gratings. The conventional design method based on only the electromagnetic grating analysis yields the theoretical highest diffraction efficiency. However, the calculated grating profiles may be too complex to be fabricated, so that such a high efficiency will not realized.

\n\t\tMicroRNAs or miRNAs are a class of small non-coding RNA approximately 21–25 nucleotides that modulate on gene expression post-transcriptionally via binding to the 3′ untranslated region (3′-UTR) of the target messenger RNA (mRNA), resulting in mRNA degradation or translational repression. The first miRNA, lin-4, was discovered by Ambro and his research group in 1993 and it was found to be related with larva development in
miRNAs are normally transcribed by RNA polymerase II from miRNA genes. This transcription leads to generate a primary miRNA transcript (pri-miRNA). Then, pri-miRNA is further cleaved by a microprocessor complex, which consists of Drosha, the double-stranded RNase III enzyme and DiGeorge syndrome critical region 8 (DGCR8), important cofactor, into a hairpin structure precursor miRNA (pre-miRNA) in the nucleus (Figure 1). The double strand pre-miRNAs with 70 nucleotides are then exported to the cytoplasm by the process of nuclear export factor exportin-5. The pre-miRNA is then processed by RNase III, Dicer, thereby generating a mature miRNA:miRNA duplex approximately 22 nucleotides in length and without a hairpin structure. The helicase enzyme cleaves miRNA duplexes into single-stranded miRNAs and incorporated into the Argonaute (AGO), TRBP and PACT proteins to form the RNA-induced silencing complex (RISC). Usually, other single strand called passenger strand or the star (*) strand will be degraded, while single strand mature miRNA is able to bind with its target mRNA and mediating translational inhibition or mRNA degradation, along with their sequence complementarity to the target [1, 3]. In plants, target mRNA will be degraded if miRNA has perfect or near-perfect complementarity to its target. In contrast to mammal, miRNAs bind to partially complementary sites in the 3′-UTRs of target mRNA, which leading to translational repression [4]. the target mRNA is either blocked (imperfect complementary) or degraded (perfect complementary) of the ribosomal translation, which sequentially impacts the cellular functions.
miRNA biogenesis. miRNA gene is transcribed by RNA polymerase II and then forming the primary miRNA transcript (pri-miRNA), which is further cleaved by the Drosha/DGCR8 complex to generate the precursor miRNA (pre-miRNA). Pre-miRNA is then exported into the cytoplasm by exportin 5/RAN-GTP and further processed by dicer to create the mature miRNA, which is loaded into RISC, which contains AGO, PACT and TRBP proteins. Mature miRNA that binding to its target mRNA by perfect complementary binding and resulting in gene suppression by mRNA degradation. The partially complementary binding of miRNA and its target mRNA, which in turn inhibit the protein translation.
Phytochemicals are major plant-derived compounds that naturally found in vegetables, fruits, medicinal plants or other plants with medicinal properties including antioxidant, anti-diabetic, anti-inflammatory, antimicrobial, antidepressant, anticancer and prevention in other chronic non-communicable diseases [5, 6, 7]. Phenolic and flavonoid compounds are the most important group of bioactive compounds and second metabolites in plants which comprise of essential molecules of human diet [6, 8]. It has been shown that bioactive compounds can modulate the endogenous miRNAs expression [1, 9, 10, 11, 12]. Recently, some studies have revealed that plant-derived miRNAs (dietary miRNAs) as new bioactive compounds in plants can affect the synthesis of endogenous miRNAs [13, 14, 15]. Strikingly, miRNAs do not function only their origins but they are able to regulate the gene expression in cross-kingdom. Therefore, bioactive compounds present in functional foods are potentially regulate endogenous miRNAs expression.
Extensive studies have been performed to understand the molecular mechanism of bioactive compounds with a positive effect on chronic diseases or non-communicable diseases such as arthritis, cancer, cardiovascular diseases, diabetes and obesity [1, 16]. Emerging evidences confirm that alteration of endogenous miRNAs expression can be influenced by bioactive compounds in functional foods [16, 17] (Figure 2 and Table 1).

Cinnamic acid derivatives can occur naturally in plants and their structure composing of benzene ring and acrylic acid group. Several compounds of cinnamic acid derivatives have been identified including artepilin C, baccharin, drupanin, ferulic acid, curcumin, caffeic acid, p-hydroxycinnamic acid, coumaric and chlorogenic acids, etc. [30, 31]. Medicinal activities of cinnamic acid derivatives have been reported such as anti-inflammatory, anti-oxidant, anti-viral, anti-microbial, anti-diabetic, neuroprotective and anti-tumor activities [30, 31, 32]. Cinnamic acid derivatives from propolis significantly induced colon cancer cell apoptosis through TRAIL/DR4/5 and/or FasL/Fas death-signaling pathways and via the upregulated miR-143 expression, resulting in decreased the target gene MAPK/Erk5 expression and its downstream target c-Myc [31]. Moreover, Li et al. demonstrated that cinnamic acid derivatives decreased gastric cancer cell proliferation through the up-regulation of miR-145 and down-regulation P13K/Akt signaling pathway [33]. Therefore, cinnamic acid derivatives have a potential as therapeutic agents for cancer.
Curcumin[(1,7-bis(4-hydroxy-3-methoxyphenyl)-1,6-heptane-3,5-dione] is well known as natural polyphenol and derived from the rhizome of turmeric or
3,3′-diindolylmethane (DIM) is a naturally active compound found in stomach, which derived from indole-3-carbinol (I3C) that present in cruciferous vegetables. DIM has been reported to regulate several miRNAs expression in cancer. Tumor suppressor miRNAs was upregulated by DIM in prostate cancer cells including let-7, miR-34a and miR-150-5p by targeting EZH2, Notch1 and AR and Ahr, respectively [46]. DIM also upregulated tumor suppressor miR-200, which led to inhibit the expression of FoxM1 in breast cancer cells [47]. miR-212/132 cluster and miR-21 were upregulated by DIM, which downregulated the expression of Sox4 and Cdc25A, respectively in breast cancer [48, 49]. Moreover, DIM upregulated let-7b, let-7c, let-7d, let-7e, and miR-200b/c expression, which led to inhibit the expression of ZEB-1, E-cadherin in pancreatic cancer cells [50]. It has been reported miR-146a was upregulated upon treated with DIM and suppressed the expression of MTA2, NF-κB, IRAK1, EGFR in pancreatic cancer cells [51].
DIM showed the modulation of miRNAs expression in other inflammatory diseases. The expression of miR-106a, miR-20b, and miR-125b-5p were increased after treatment with DIM and suppressed the expression of IRAK4 and TNF-α to limit responses to TLRs activated by LPS in acute liver failure (ALF) animal model [52]. DIM significantly upregulated miR-200c, miR-146a, miR-16, miR-93, and miR-22 in brain CD4+ T cells and inhibited the expression of cyclin E1 and B-cell lymphoma-2 in experimental autoimmune encephalomyelitis (EAE), a murine model of multiple sclerosis [53].

EGCG showed the protective effect against myocardial ischemia/reperfusion (I/R) injury through up-regulation of miR-384-mediated autophagy by targeting Beclin-1 via activating the PI3K/Akt signaling pathway [62]. EGCG also demonstrated the anti-arthritic effects by inhibited IL-1β-induced ADAMTS5 expression and up-regulated the expression of miR-140-3p in osteoarthritis chondrocytes [63]. EGCG treatment has potential role of preventing toxin-induced fibrosis by suppression of osteopontin expression and up-regulation of miR-10b, miR-181a and miR-221 in liver hepatocellular carcinoma cells [64].
Genistein belongs to isoflavone family and presents in soybeans with antiangiogenic, anti-metastasis, anti-inflammatory, anti-oxidant, cell cycle arrest and induction of apoptosis effects [65]. Genistein can regulate the expression of miRNAs in several call types [65]. It has been reported that treatment of genistein up-regulated miR-23b and inhibited breast cancer cell growth [66]. Genistein also exhibited anti-tumor effect by down-regulated miR-1260b and targeting sRRP1 and Smad4 through DNA methylation or histone modifications in prostate cancer cells [67]. The same research group reported that miR-1260b was highly expressed in renal cancer cells and miR-1260b was down-regulated in genistein treated renal cancer cells [68]. The treatment of miR-1260b inhibitor inhibited the expression of its target genes, sFRP1, Dkk2, Smad4 [68]. Treatment with genistein induced non-small lung cancer cell apoptosis, caspase-3/9 activation and inhibited cell proliferation via up-regulation of miR-27a -mediated MET signaling [69]. Co-encapsulate miR-29b with genistein in hybrid nanoparticles (GMLHN) has been studied to treat effectively in non-small lung cancer cell and GMLHN showed the anti-proliferative effect by down-regulation of phosphorylated AKT (pAKT) and phosphorylated phosphoinositide-3 kinase (p-PI3K) [70].
Genistein promoted myoblast proliferation and differentiation through down-regulated miR-222 expression, resulting in increased expression of its target genes, MyoG, MyoD, and ERα [71]. Interestingly, genistein up-regulated miR-451 expression and inhibited IL1β expression and inflammation in chronic liver disease nonalcoholic steatohepatitis (NASH) mice model [72].
Quercetin is bioactive flavonoids that can be found in fruits and vegetables including onion, kale, apple, many berries, citrus fruits and tea [73]. Anti-cancer, anti-inflammatory, antioxidant, anti-diabetes, anti-atherosclerosis and anti-viral effects have been reported in different in vitro studies for quercetin [74]. Several studies have focused on quercetin and miRNAs modulation for therapeutic approaches. miR-200b-3p was up-regulated in pancreatic cancer cells when treated with quercetin, resulting in inhibition of self-renewal and decrease of proliferation through Notch1 signaling pathway [75]. Quercetin significantly inhibited breast cancer cell proliferation and invasion via up-regulated miR-146a expression and targeting EGFR [76]. Quercetin inhibited cell viability, migration and invasion by up-regulated miR-16 and targeting HOXA10 in oral cancer cells [77]. In addition, quercetin decreased oral cancer cell viability and increased cell apoptosis via miR-22/WNT1/β-catenin pathway [78].
Recently, quercetin modulated 34 miRNAs expression (5 upregulated and 29 downregulated) and novel miR-97, miR-298, miR-2218, miR-1502, and miR-2117 were identified in pheochromocytoma of the rat adrenal medulla that responded for protective effect against oxidative stress through PI3K-AKT signaling pathway [79]. Treatment of quercetin inhibited proliferation of endometriosis through up-regulated miR-503-5p, miR-1283, miR-3714 and miR-6867-5p by targeting CCND1 [80]. TGFβ1 is a fibrosis inducer and quercetin significantly down-regulated miR-21 and TGFβ1 and up-regulated miR-122 in liver fibrosis [81]. Protection of cardiomyocyte against hypoxia caused insults of quercetin has been reported by up-regulation of miR-199 mediated sirt1 expression and AMPK phosphorylation [82].

Sulforaphane has potential to inhibit hepatic fibrosis by downregulating miR-423-5p in hepatic stellate cell [99]. Sulforaphane showed the protective effect in microglia-mediated neurotoxicity by inhibited LPS-induced expression of inflammatory miRNA, miR-155 [100].
Several evidences demonstrated the direct modulation of cellular signaling pathways by dietary compounds could decrease the risk of chronic diseases [101]. Interestingly, it has been reported that small non-coding RNA including miRNAs can be transferred across Kingdoms, for example dietary miRNAs have been found in human body fluids and these circulating miRNAs are likely to regulate human gene expression [15, 102, 103, 104, 105, 106, 107]. The uptake of plant derived miRNAs could be in the form of raw and cooked plants in capable of stability forms [107, 108]. Due to high temperature cooking process, low pH and enzymes in digestive tract as well as enzymes in blood circulation, miRNAs might be destroyed before their functions with target mRNAs [15]. Strikingly, GC base content, 2’-O-methylation on the 3′-terminal, unique nucleotide sequence of dietary miRNAs and extracellular vesicles (exosome and microvesicle) are preventive features of plant derived miRNAs in harmful conditions [109, 110, 111, 112, 113, 114].
There are numerous studies to support the functional roles of dietary miRNAs in cross kingdom gene regulation. Dietary miRNAs and human gene regulation.
The abundantly expressed miRNA in dietary green vegetable, miR156a which was detected in human serum and targeted the junction adhesion molecule-A (JAM-A) [118]. The JAM-A was up-regulated in atherosclerotic lesions from cardiovascular disease patients and miR156a could suppressed inflammatory cytokine-induced monocytes adhesion by targeting JAM-A [118]. The very recently report using a computational approach to predict the potential target of rice miRNAs including miR156-5p, miR164-5p, miR168-5p, miR395-3p, miR396-3p, miR396-5p, miR444-3p, miR529-3p, miR1846-3p, miR2907-3p, which can bind to the human mRNA [119]. It has been widely known that functional foods and their bioactive compounds have the capacity for human health benefits. To date, miRNAs have been shown a significant effect on gene expression and modulate the cellular biological functions in physiological and pathological conditions. There is emerging evidence suggesting that dietary bioactive compounds can be effective in human diseases as a result of altering miRNAs expression levels, resulting in modulation of cellular signaling pathway. Additional research the possibility of bioactive compounds for developing as novel drugs with less side effects is required

\n\nYour manuscript will be sent to Straive, a leader in content solution services, for language copyediting. You will then receive a typeset proof formatted in XML and available online in HTML and PDF to proofread and check for completeness. The first typeset proof of your manuscript is usually available 10 days after its original submission.
\n\nAfter we receive your proof corrections and a final typeset of the manuscript is approved, your manuscript is sent to our in house DTP department for technical formatting and online publication preparation.
\n\nAdditionally, you will be asked to provide a profile picture (face or chest-up portrait photograph) and a short summary of the book which is required for the book cover design.

The upper airway has an important role in transporting air to the lungs. The airway changes in size, shape, and position throughout its development from the neonate to the adults. Knowledge of the functional anatomy of the airway in these forms the basis of understanding the pathological conditions that may occur. The upper airway extends from the mouth to the trachea. It includes the mouth, the nose, the palate, the uvula, the pharynx, and the larynx. This section also describes the functional physiology of this airway. This cognitive impairment affects the long-term prognosis and has been shown to be associated with long-term disability, higher health care costs, and even increased mortality. On the other hand, clinical research on POCD is in its infancy, the condition has not been clarified, and since no strategy for management is currently available, it is imperative to develop specific methods for prevention and management. Although its pathogenesis involves various factors, accumulating evidence suggests that surgery elicits an inflammatory response in the hippocampus, a brain area closely related to cognitive function, playing a key role in the development of POCD. Several studies suggest that age-related phenotypic change of microglia is associated with pathogenic neuroinflammation, and more importantly it may be modifiable. In this chapter, we discuss the current overview and preclinical highlights regarding POCD. The management of intravenous fluids is controversial, but the current trend is the restricted use of hydrosaline solutions. It includes the mouth, the nose, the palate, the uvula, the pharynx, and the larynx. This section also describes the functional physiology of this airway. Identifying the problem causing the patient’s respiratory failure helps in making the decision to intubate. In fact, the clinician must be fast and self-confident when deciding on intubation. It is difficult to decide in some complex situations. It is very important to evaluate the patient, according to clinical status, age, and comorbidity, and to determine urgent intubation need. In non-diagnostic cases, further research is needed to investigate the causes of the condition such as hypoxia/hypercapnia resulting in patient respiratory distress. Arterial blood gas analysis will facilitate our decision to make intubation. The proper selection and correct preoperative assessment of patients are the key to stay away from unwanted events. Preanesthesia evaluation is mandatory in each patient and must include clinical history, complete physical examination, and routine and special laboratory tests in patients with associated pathologies. Anesthetic management is based on these results, type of surgery, experience of the anesthesiologist, and the operating environment. The anesthetic technique can be local, regional, or general with standard noninvasive monitoring. It is recommended that an anesthesiologist be present in all plastic surgery procedures. Complications are usually the result of moving away from the guidelines already established for an excellent practice or the result of sentinel events rather than human errors. Pulmonary embolism is probably the most feared complication, with soft tissue infections being the most frequent complication in plastic surgery. Less common complications include arrhythmias, overhydration, allergies, bleeding, skin necrosis, dehiscence of wounds, brain damage, and dead. During this period, Prof. Iadarola familiarized himself with the conventional techniques used in column chromatography, spectrophotometry, manual Edman degradation, and electrophoresis). Since 1995, he has been working on: i) the determination in biological fluids (serum, urine, bronchoalveolar lavage, sputum) of proteolytic activities involved in the degradation processes of connective tissue matrix, and ii) on the identification of biological markers of lung diseases. In this context, he has developed and validated new methodologies (e.g., Capillary Electrophoresis coupled to Laser-Induced Fluorescence, CE-LIF) whose application enabled him to determine both the amounts of biochemical markers (Desmosines) in urine/serum of patients affected by Chronic Obstructive Pulmonary Disease (COPD) and the activity of proteolytic enzymes (Human Neutrophil Elastase, Cathepsin G, Pseudomonas aeruginosa elastase) in sputa of these patients. More recently, Prof. Iadarola was involved in developing techniques such as two-dimensional electrophoresis coupled to liquid chromatography/mass spectrometry (2DE-LC/MS) for the proteomic analysis of biological fluids aimed at the identification of potential biomarkers of different lung diseases. Biochemistry examines macromolecules - proteins, nucleic acids, carbohydrates, and lipids – and their building blocks, structures, functions, and interactions. Much of biochemistry is devoted to enzymes, proteins that catalyze chemical reactions, enzyme structures, mechanisms of action and their roles within cells. Biochemistry also studies small signaling molecules, coenzymes, inhibitors, vitamins, and hormones, which play roles in life processes. Biochemical experimentation, besides coopting classical chemistry methods, e.g., chromatography, adopted new techniques, e.g., X-ray diffraction, electron microscopy, NMR, radioisotopes, and developed sophisticated microbial genetic tools, e.g., auxotroph mutants and their revertants, fermentation, etc. More recently, biochemistry embraced the ‘big data’ omics systems. Initial biochemical studies have been exclusively analytic: dissecting, purifying, and examining individual components of a biological system; in the apt words of Efraim Racker (1913 –1991), “Don’t waste clean thinking on dirty enzymes.” Today, however, biochemistry is becoming more agglomerative and comprehensive, setting out to integrate and describe entirely particular biological systems.
