Mirjana Rajer

Assistant Professor Mirjana Rajer, MD, PhD, graduated from the Faculty of Medicine at the University of Ljubljana in 2001. For two years, she worked at the University Clinic Golnik, Slovenia. Currently, she works at the Institute of Oncology Ljubljana. Her main clinical work consists of treatment of lung cancer and mesothelioma patients. She is also involved as principal and co-investigator in several clinical trials of lung cancer, palliative care, and communication with cancer patients. She serves as an editor for national and international journals and is author of numerous published papers, book chapters, and books. Being a student mentor, Dr. Rajer is actively involved in education. She is an active member of the European Organisation for Research and Treatment of Cancer (EORTC), European Society for Medical Oncology (ESMO), International Association for the Study of Lung Cancer (IASLC), and the Central European Initiative against Lung Cancer.

This book is a collection of in-depth chapters on many aspects of contemporary cancer treatment. Written by experts worldwide, each chapter provides a detailed summary of the state-of-the art knowledge in the area, with extensive references and clear and informative diagrams. The volume is divided into two sections: “Basic Science” and “Clinical Challenges.” The five chapters in the first section cover MicroRNA, the role of angiogenesis in the tumor microenvironment, microbial metabolites in the gastrointestinal microenvironment, the role of dendritic cells in anti-tumor immunity, and challenges of current cancer biomarkers. The two chapters in the second section cover pediatric CNS tumors and the role of sentinel node biopsy in endometrial cancer. Prognostic and Predictive Value of PD-L1 in Patients with Lung Cancer

Improved understanding of the molecular mechanisms has led to identification of checkpoint signalling and development of checkpoint inhibitors in the treatment of many cancers, including lung cancer. To be able to select the patients who benefit most from checkpoint inhibitors, predictive biomarkers are needed. Currently, the only predictive biomarker that has been approved in clinical use is PD-L1, the ligand of the inhibitory T-cell checkpoint PD-1. The use of PD-L1 as a predictive biomarker is confounded by multiple unresolved issues, from testing issues (e.g., cut-off values for positivity) to clinical use (e.g., the response to anti–PD-1 and anti–PD-L1 antibodies in patients without any expression of PD-L1). Even more open questions exist in the evaluation of PD-L1 as a prognostic biomarker. In the future, we expect that an improved understanding of immune system, tumor microenvironment, mechanism of action of immunotherapeutic drugs, and PD-L1 testing methods will elucidate the value of PD-L1 as a prognostic and predictive biomarker in detail.
