Giovanni Palleschi

Giovanni Palleschi (MD, Ph.D.) graduated in 1997 and specialized in urology in 2002 at Sapienza University of Rome. In 2002 he won a prize for research on anatomo-clinical correlations of urinary disorders in patients with multiple sclerosis, and in 2011 he achieved a physician doctorate in human anatomy, dermatology, and plastic surgery. He has been a member of the Scientific Committees of the Italian Society of Urology, the Italian Society of Urodynamics, and the European Association of Urology. He participated as a co-investigator in numerous clinical trials. He is the author of 70 publications edited on PubMed and scientific books. He participated in academic teaching activities at the Sapienza University of Rome. At present, he is a consultant urologist for the Nefrocenter Group (Italy).

Among inflammatory disorders of the urinary tract, cystitis is the most common. A significant increase in the prevalence of this condition is reported by epidemiological investigations, and its diagnostic and therapeutic approach often represents a challenge. Bacterial cystitis is becoming difficult to treat, especially if chronic or recurrent, due to the increase of drug resistance among the responsible pathogens. Chronic non-infectious cystitis is also difficult to treat because its pathophysiologic mechanisms are still not completely understood. This book provides a comprehensive overview of bladder inflammation, focusing on etiopathogenesis, pathophysiology, and diagnostic and therapeutic approaches to the main types of infectious and non-infectious cystitis in both adults and children. It also addresses radiation cystitis as well as iatrogenic cystitis.

Quality of Life in Neurogenic Bladder Patients and Improvement after Botulinum Toxin Injection

Various neurogenic conditions may determine an altered function of the bladder and urethral sphincters leading to urinary symptoms. Among these symptoms, loss of urine is considered the most bothersome, and recent literature has proposed that urinary incontinence and poor quality of life should be considered as associated conditions. Urinary incontinence is responsible for reducing the enjoyment of life, including loss of self-confidence and limitation of social activity; it is also associated with anxiety, depression, and deterioration in sexual life, and reduced physical activity. Conservative treatments represent a first-line therapeutic approach to neurogenic urinary disorders, followed by oral medications. However, these treatments often do not provide complete recovery from symptoms, especially from urinary incontinence. Onabotulimun toxin A has proven to be safe and effective for treating neurogenic urinary incontinence and its use is associated with a strong improvement of patients’ quality of life. Furthermore, this treatment lowers the risk of severe complications to the upper urinary tract, reducing the need for hospitalization. Quality of life improvement as well as clinical efficacy must be considered targets of the therapy, and the use of onabotulimun toxin A for treating neurogenic urinary incontinence reaches both these goals.
