Prof. Ju-Seop Kang received his medical degree from Hanyang University College of Medicine, South Korea. He is currently a professor and director of the Department of Pharmacology and Clinical Pharmacology at the same university. He is also on the committee for the Department of Research Integrity at Hanyang University and a member of the appraisal committee of the Korea Medical Dispute Mediation and Arbitration Agency of the Ministry of Health and Welfare. He has published more than 120 papers in peer-reviewed international journals and 10 book chapters. He is a reviewer for five international journals and on the editorial board of several others.

Non-alcoholic fatty liver disease (NAFLD) is characterized by hepatic steatosis that develops in the absence of competing for liver disease etiologies such as alcohol consumption, monogenic hereditary conditions, or iatrogenic causes. The pathogenesis of NAFLD is multifactorial and its understanding is still incomplete. Although knowledge of the cellular and molecular mechanisms underlying disease development and progression has grown signiﬁcantly in recent years, the exact contribution of environmental and genetic factors as well as that of extrahepatic and intrahepatic events in determining the disease phenotype remains ill deﬁned. This book discusses topics highly correlated with NAFLD such as the regulation of iron metabolism, the role of the enzyme heparanase in liver steatosis, transcriptional regulation by ERR and its role in pathogenesis, hepatic lipid homeostasis, therapeutic approaches to NAFLD, and pathogenesis and significance of HDL as a molecular modifier in NAFLD.

Noninvasive Diagnostic and Prognostic Assessment Tools for Liver Fibrosis and Cirrhosis in Patients with Chronic Liver Disease

Liver fibrosis, that is, excessive accumulation of extracellular matrix protein, occurs and is the wound‐healing response and common final pathway of various chronic liver diseases. Advanced hepatic fibrosis caused by chronic liver inflammation eventually progresses to cirrhosis, and prognosis and management of chronic liver diseases depend on the fibrotic severities. Therefore, the early and precise evaluation of severity and status of liver fibrosis provides useful information for diagnosis as well as treatment planning and treatment efficacy and prognosis. Although invasive liver biopsy is the gold standard to assess the nature and severity of hepatic fibrosis, it has several recognized limitations including sampling error and inter‐observer variability in interpretation and staging. Furthermore, the dynamic process of fibrosis resulting from progression and regression is difficult to capture with biopsy alone. Therefore, alternative, simple, reliable, and noninvasive direct and indirect serum markers able to predict the presence of significant fibrosis or cirrhosis in patients with chronic liver disease with considerable accuracy were needed. The hepatology experts are actively researching noninvasive methods of fibrosis quantification. The aims of this chapter were to review the nature and limitations of the several noninvasive methods for the assessment of presence and severity of liver fibrosis in patients with chronic liver disease.
